As part of the process, a one-time medical clearance is required to ensure that the selected IV therapy, vitamin injections, or weight loss treatment is safe and appropriate for you. This clearance form can be found on the appointment request page, just click the green tab that says medical clearance form, and fill out the necessary information. You will receive an email response letting you know if you have been approved or denied for treatment. The medical clearance form will be completed before the treatment is administered, ensuring a personalized and secure experience tailored to your health needs.
